Your Care Coordinator will help with:

- Accessing resources, social support and activities in your area

- Communicating with doctors, nurses and other providers
- Coordinating transportation to your appointments

- Finding employment or returning to school
You may be eligible to receive Care Coordination Services if you are:
- A Nassau County resident
- Medicaid eligible
- Diagnosed with two chronic health conditions

Our nutritionists will help you:
- Develop healthy eating habits
